One of the common conditions we help with is IT Band Syndrome that sharp, stubborn pain on the outside of the knee that stops runs in their tracks.

The iliotibial (IT) band is a thick band of connective tissue running down the outside of the thigh, from the hip to just below the knee. It helps stabilize the knee with every stride. When the IT band and the muscles attached to it become excessively tight, the tissue at the outside of the knee becomes compressed and irritated with each bend and straighten of the leg — and IT band syndrome is born.

It’s one of the most common overuse injuries we see in runners and cyclists. The root causes usually live upstream: tight or overworked hip muscles (particularly the tensor fasciae latae, which feeds into the IT band), weak glutes that allow the hip to drop and the knee to drift with each stride, rapid training increases, and lots of downhill or cambered-road running.

What are the symptoms of IT Band Syndrome?

The classic symptom is sharp or burning pain on the outside of the knee that appears at a predictable point in your run — often a specific mileage — and gets worse the longer you continue. Downhill running is a notorious trigger. In more irritated cases, the pain shows up even when walking, especially on stairs.

Many people also feel tightness or aching along the entire outside of the thigh, tenderness when pressing on the outer knee, and sometimes a snapping sensation as the tissue moves over the bone. Unlike some injuries, IT band pain often vanishes at rest — then returns like clockwork the next time you run, which is exactly what makes it so maddening.

Runners often try to push through it, but IT band syndrome rarely yields to willpower. Without addressing the tightness and imbalances driving it, the flare-up distance tends to shrink — from mile five, to mile two, to the end of the street.

What Is Our Approach To Fixing / Treating IT Band Syndrome?

The key insight about IT band syndrome: the outer knee is where it hurts, but it’s usually not where the problem lives. We assess the whole chain — your hip muscles, glutes, quadriceps, and the IT band itself — to find the tension and imbalances that are overloading the outside of your knee.

Hands-on treatment focuses on releasing the muscles that feed tension into the IT band — especially the tensor fasciae latae and outer hip — along with the quadriceps and hamstring tissue that binds against the band along the thigh. Foam rolling the band at home rarely solves this on its own; skilled, targeted work on the right structures is what makes the difference.

As the tension through the outer thigh eases, compression at the knee drops and the irritated tissue can finally calm down. We’ll then guide your return to running and give you the stretches and strength pointers — particularly for the glutes — that keep the IT band from tightening back up.
